Reverse shoulder arthroplasty in rheumatoid arthritis: survival and outcomes

چکیده
BackgroundDespite its potential biomechanical advantages, reverse shoulder arthroplasty (RSA) is still considered to be particularly high risk in rheumatoid patients who are osteoporotic and immunodeficient. Our purpose was report prosthesis survival, complications, outcomes of RSA with arthritis (RA) at minimum 5-year follow-up.MethodsWe conducted a retrospective multicenter study including 65 consecutive primary RSAs performed 59 RA between 1991 2010. We excluded previous failed anatomic arthroplasty. Age surgery averaged 69 years (range, 46-86 years). A structural bone grafting 18 cases (45%), using the humeral head 15 (BIO-RSA technique), iliac crest 2 (Norris an allograft 1 case. The mean follow-up 92 months 60-147 months) or until revision surgery.ResultsRevision-free survivorship, Kaplan-Meier curves, 96% 7 years. Two had surgeries for infections, associated glenoid loosening No recorded. adjusted Constant score improved from 36% ± 23% preoperatively 90% 26% postoperatively, Subjective Shoulder Value 21% 13% 85% 12%, respectively (P < .001). Active anterior elevation increased 65° 43° 132° 27°, active external rotation 10° 26° 22° internal buttocks waist Stable fixation baseplate achieved all (including 6 end-stage RA), we did not observe graft nonunion resorption. Preoperative radiologic pattern (centered, ascending, destructive), presence acromial fractures tilt (4 cases, 10%), scapular notching (55%) on final radiographs were found influence complication rate. Patients absent/atrophied teres minor lower functional results. Overall, 95% satisfied procedure.ConclusionRSA safe effective procedure treatment patients, low complications rate revision, regardless presentation stage disease. Rheumatoid undergoing RSA, without grafting, can expect revision-free survival 7-year follow-up. offers benefit solving key problems encountered shoulders: destruction rotator cuff deficiency. Despite surgery. Revision-free procedure.

Reverse Shoulder Arthroplasty in Rheumatoid Arthritis: A Systematic Review
BACKGROUND Anatomical shoulder replacement for rheumatoid arthritis (RA) is complicated by a high incidence of rotator cuff tears and glenoid erosion. This can lead to poor function and early failure. Reverse shoulder arthroplasty (RSA) has gained popularity as an alternative. This systematic review attempts to further define the role of RSA in RA. METHODS A systematic review identified seven...
متن کاملReverse Shoulder Arthroplasty in the Treatment of End-Stage Rheumatoid Arthritis of the Shoulder
Rheumatoid arthritis (RA) of the shoulder affects both bony and soft-tissue structures around the glenohumeral joint and destroys the rotator cuff, and leads to loss of function, instability, and chronic incongruency of the joint. Lehtinen et al. [1] 2000 reported a natural history of 148 RA shoulders with a 15-year follow-up. The result was upward migration, medialization, and flattening of th...
متن کاملRheumatoid Arthritis: Early diagnosis and treatment outcomes
Rheumatoid arthritis (RA) is an inflammatory progressive disease which in the absence of appropriate treatment can lead to joint destruction and disability. Prognosis of RA may be predicted based on the presence of some clinical and laboratory evidences.New criteria for classification of RA provides opportunity for earlier treatment. Initiation of treatment particularly by combination of DMARDs...
متن کاملReverse Shoulder Arthroplasty in Patients with Rheumatoid Arthritis: A Systematic Review
BACKGROUND The purpose of this study was to describe the pathoanatomy of patients diagnosed with rheumatoid arthritis and rotator cuff deficiency and report their outcomes following reverse shoulder arthroplasty. METHODS Twenty-one shoulders were evaluated prospectively. Nine had no prior surgery, 9 had a failed rotator cuff repair, and 3 had a failed arthroplasty. Patients were followed for ...
متن کاملThe results of shoulder arthroplasty in patients with rheumatoid arthritis.
We have performed a clinical and radiological analysis of 105 shoulder arthroplasties in patients with rheumatoid arthritis. The clinical results showed improvements in the Constant-Murley and Association of Shoulder and Elbow Surgeons score of 21 and 35, respectively. Both were statistically significant (p < 0.001). This improvement was maintained over a period of 8.8 years. There was no stati...
